This is a known issue seen in Pro Tools 6.2cs2 on WinXP that occurs when an Avid client's OMFI Media Files folder on a Unity volume contains illegal characters.
For example, a Mac OS X Avid system will auto create a folder named, "OMFI Media Files\John's Computer". The ( ' ) being the illegal character. To resolve, go to the File Sharing preference for all offending OS X Unity clients and rename the computer's name so that there are no illegal PC characters.
At this point, you will most likely need to re-export the OMF from the Avid.

This is a known issue in AVoption|XL 6.x that only occurs when opening pre-6.x created sessions when some video files are missing.
Two workarounds exist:
1. Open the session in 5.x and delete the video track, save and open the audio only session in 6.x. Then reimport the video in 6.x.
2. In AVXL 6.x, open the session with the BOB turned off which will remove the video track. The video can then be reimported.

This is a known issue in Pro Tools and AVoption|XL 6.x when links to files have been broken (i.e. media files have been moved from their original location).
When opening pre 6.1 sessions, it is important to make sure the video files reside in a Video Files folder that is placed inside a folder with the session name. Audio files must be in an Audio Files folder inside a folder with the session name. The session folders must be at the top level of the audio and video drives, respectively. Failure to do this may result in missing files that can NOT be relinked to from the Relink Window.

Pro Tools is not seeing the Meridien hardware. Try the following:
1. Power cycle the Meridien BOB
2. Reseat Meridien cable at both ends
3. Reseat the Meridien PCI card
4. Swap the slot order of the Meridien PCI card and ATTO host bus adapter (fibre or SCSI).
5. Trash preferences
6. Repair disk permissions on the boot drive (OSX only)
7. On WinXP, trash volume.ddb files for all mounted Unity MediaNet volumes. Note - these live on the boot drive. Try unmounting some Unity volumes.
8. Make sure that Programs are prioritized above Backgournd Services & System Cache in Mycomputer -> properties -> advanced -> performance -> settings - > advanced.
9. Try unmounting all mounted SCSI, Firewire, and ATA drives.
Also, confirm that AVoption|XL is authorized via iLok and that the iLok is connected and being seen by Apple System Profiler.

This is a known issue in AVoption|XL 6.2.2 and higher when running a G5 and attempting to digitize video to drives connected to a SCSI card installed internally in the G5. This will also occur if attempting to record video to an SATA drive.
For single SCSI card AVXL configurations in G5's, the ATTO UL3D SCSI card MUST be installed in the expansion chassis (in the slot next to the Meridien card in the chassis' 2nd PCI bus). Use one channel of the SCSI card for audio, the other for video. Single SCSI card configurations have been tested for 3:1 video or higher.
For better performance, 2 SCSI cards can be used. Two SCSI cards are required for 2:1 or 1:1 video. In this configuration, use the SCSI card in the CPU for audio, and the card in the chassis for video.

The Video Engine for BOB ("DIO Video Engine") differentiates between three types of buffer underruns. Here's a list and the accompanying error dialog that gets posted when they happen.
1) Disk Buffer Underrun - this indicates that we couldn't read data off disk fast enough to play the video. "Video playback stopped due to a disk fifo buffer underrun."
2) Software Decompression Buffer Underrun - this indicates that there was not sufficient CPU cycles to decompress video fast enough to keep playback going. "Video playback stopped due to a software decompression buffer underrun."
3) DIO buffer underrun - this indicates that our Video Engine kept up with all of it's deadlines but the tasking model within Avid's DIO software got behind or the FireBOB hardware reported an underrun. "Video playback stopped due to a hardware or driver buffer underrun."
With uncompressed video, the probabilities that we'll get a disk buffer underrun are much higher.
