There are several issues that could be causing this. Most cases are resolved in Pro Tools 6.9.1cs3 (Mac OS 10.3.x TDM), 6.9.3cs2 (Mac OS 10.4.x & Win XP TDM), 6.9cs4 (Mac OS 10.3.x & Win XP LE), and 6.9.2cs4 (Mac OS 10.4.x LE). We also recommend that Avid users upgrade to Media Composer Adrenaline 2.1.7, Xpress Pro 5.1.7, and News Cutter Adrenaline & News Cutter XP 6.1.7 and higher.
These issues will be resolved in a future version of Media Station|PT and Avid Studio Bundle.

Fixed in Pro Tools 6.9cs3 Win XP and Mac LE, 6.9.2cs3 Mac LE, 6.9.1cs2 Mac TDM, and 6.9.3cs1 Mac and Win XP TDM.
The "fine-tune breakpoint" Control key functionality has now been restored to the pre-Pro Tools 6.9 behavior. The key command for "Write Level Constrained to Next Breakpoint" (new to 6.9) has been changed from "Control-click" to "Alt-Shift-click." The key command for "Write Level Constrained to Previous Breakpoint" remains "Control-Alt-click."

This will occur if Automatic Delay Compensation is enabled in the Playback Engine and there are plug-ins inserted on one or more tracks in which the sample delay for the track is 2566 or greater.
Workaround: temporarily make some or all plug-ins inactive while digitizing video (Ctrl-Win-Alt click on the plug-in), or set Automatic Delay Compensation to None in the Playback Engine (you will be prompted to close and reopened the session).
In one case, adding 1 frame of pre roll to the transport resolved the problem.

This can occur on a system that was previously running AVoption|V10 6.4 if the old "Digidesign AVoptionDNA" software was not uninstalled. Even if Device Manager is correctly showing that the new 1.0.6.3 AVoption|V10 driver is loaded, it can still be bound to the older driver. Go to Start>Control Panel>Add or Remove Programs, Remove Digidesign AVoptionDNA and Restart. Launch Pro Tools and watch the splash screen to see if it properly initializes the video engine.
If the video engine still doesn't initialize, it may be necessary to go to Device Manager>Sound Video and Game Controllers, right click on Avid AVoption|V10 and select Update Driver. Click on Have Disk and navigate to C\ProgramFiles\Digidesign\Drivers and select Flamethrower.

When installing the Media Station user data to a custom location, that same exact location must be entered when launching the application for the first time.
To resolve this issue if the above correct procedure wasn't followed:
1. Uninstall Media Station. Manually delete any Media Station files located at C:\Program Files\Avid\AvidMediaStationPT\ .
2. Go to Start>Run and type in "regedit" and hit Okay. Navigate to HKEY_LOCAL_MACHINE\SOFTWARE\Avid Technology\Avid Media StationPT\1.6 and make sure that the Data Location file has been deleted. If it still exists, right click on Data Location and choose Delete. If no reg keys exist for Media Station at this location, that is okay.
3. Reinstall Media Station following the procedure in the Media Station|PT guide. If choosing a location other than the default for settings, project and user data, note the exact path that is chosen for each.
4. When launching Media Station|PT for the first time, you will be prompted to enter the exact path for settings, project and user data. Enter the exact path that was chosen at the time of installation, plus you must also add "\Avid MediaStationPT\" to the end of your path. To confirm the exact path, follow step 2 above to open the registry and find the Data Location file.
If Pro Tools prompts about expired Dynamics, EQ, and Signal Generator plug-ins upon launch after reinstalling Media Station, you must uninstall the Digidesign Shared Plug-ins from Control Panel>Add or Remove Programs, then reinstall Digidesign Shared Plug-ins from the Pro Tools install CD.

In one case, this was solved by uninstalling Media Station and Pro Tools 6.9 and reinstalling Media Station, then Pro Tools. Always follow the instructions in the Avid Video Peripherals guide when installing Media Station and V10.

Media Station|PT 1.6 installs older versions of the Digidesign Dynamics II, EQ II, and Signal Generator plug-ins. If Pro Tools is installed after Media Station, it will in most cases, update these plug-ins to the correct version.
If you're seeing this problem, go to Control Panel/AddRemove Programs and uninstall the Digidesign Shared Plug-ins. Then go to the Pro Tools 6.9 Win XP TDM install CD, navigate to the Pro Tools Installer folder/Digidesign Shared Plug-ins and run the Setup.exe in that folder.
Note: it is not necessary to run the Digidesign Sharded Plug-ins installer in a normal Pro Tools install. The Pro Tools installer will also install these same plug-ins.

Yes. Leaving this enabled can lead to permission problems when sharing files between multiple users and systems. To disable, go to Control Panel > Folder Options. Click on the View tab, scroll down, and deselect Use Simple File Sharing. Then click Apply.
