DigiTest 5.1.1b1 supports the 2001 G4 5 slot Macs.
DigiTest 5.1 Does not support the 2001 G4 5 slot Macs.
DigiTest 5.0.2 supports the G4 and displays/tests Digi cards.
DigiTest 5.0.1 supports the G4, but won't display Digi cards in it.
DigiTest 5.0 doesn't support the G4.

This was in direct relation to a hidden hardware conflict. Removing the Sound Blaster Live Platinum audio card resolved the issue.

DigiTest 5.0.2 does not support the G4 dual processor machines.
Use DigiTest 5.1.1b1. Located in Pro Tools 5.1 Installer > Beta/Bug Fix Software > DigiTest Beta - New 5-Slot Macs folder.
DigiTest does test the SCSI chip on a Disk I/O by turning it on and off so this error can indicate a problem with this chip.

Only 2 "D24 type' cards are supported in the same system.
D24 Audio cards and Mix I/O cards are considered 'D24 type' cards.
User had a Mix Farm, D24, Mix I/O, 2nd Mix I/O, & DSP Farm.

- Verify that the Digi 001 card is firmly seated in the PCI slot (it can easily pop out when snapping on the I/O connector cable if the user has not screwed in the card).
- Verify that the I/O cable is seated at both the card and Interface.
The red ADAT optical OUTPUT light (ADAT light pipe) gets its power from the PCI slot. If the card is not firmly seated, it will NOT be lit.
Running DigiTest verified that the card was not present. DigiTest did not see any card in the PCI slot but did see a SCII card (that was seated correctly). Also , as a quick check, if the card is seated correctly, the little red ADAT optical OUTPUT light will be lit.
