This can be caused by applications that are running in the background like virus scanners, graphic card utilities or go!zilla etc.
Solution:
Go to Start -> execute and type "msconfig"
Disable all application from the autostart menu that are not necessary and restart the system.

When using RTAS plug-ins on TDM systems, you may encounter DAE -6031 or -9128 errors. In these instances, you can increase
the H/W Buffer Size and CPU Limit percentage in the Hardware Setup dialog (refer to the Pro Tools Reference Guide for
details) to get better system performance. With slower CPUs, you may also need to reduce the number of RTAS plug-ins used
in the session to get acceptable results.
NOTE: In addition to slower screen redraws and UI responsiveness, larger Hardware Buffer sizes can affect automation accuracy for plug-in parameters and mute data, as well as timing for MIDI tracks.

-Check your DAE Playback Buffer (usually set to 2) and the H/W Buffer settings. The higher they are, the more out of sync your MIDI will be in relation to your audio.
-Adjust the "Global MIDI Playback Offset" in the Setups>preferences>MIDI window.

1.Place the Axis Panner personality file into the Controllers folder. The Controllers folder is located in the System/DAE folder.
2. Launch the Axis software applet. Choose preferences from the edit menu. Place a check mark next to 'Use Gallery Joystick'. Note - the Axis panner applet needs to be launched before launching Pro Tools.
3. Launch Pro Tools while keeping the Axis applet running in the background. Choose Peripherals from the Pro Tools Setups menu and then choose the MIDI Controllers tab. Choose Axis Panner from the 'Type' pulldown menu and then choose OK.
4. Go back to the Axis Applet and choose preferences. You will now be able to set various Pro Tools functions to the buttons on the Axis panner.
5. You should now be able to use the Gallery Axis Panner to control the Pro Tools surround panner.
This beta personality file ships in the beta/bug fixes software folder of the Pro Tools 5.1.1 CD.

DAE 5.1.1 CS 1 contains two fixes:
1. Support for Removable/"Hot-Swap"Media for Pro Tools TDM Mac & Pro Tools LE Mac. Adding removable SCSI, hot-swappable SCSI or Firewire media to a Pro Tools 5.1.1-based system or earlier, requires Pro Tools to be quit and re-launched for the media to be correctly recognized and accessed. DAE 5.1.1 CS 1 includes a fix that allows these types of media to be correctly recognized and accessed by Pro Tools when the application is already running. Please note that while Digidesign does not anticipate problems when using removable SCSI, "hot swappable"SCSI or Firewire drives with Pro Tools, these devices have not been fully qualified for use with Pro Tools at this time. For full details on qualified media for Pro Tools, please visit the compatibility section of Digidesign’s web site at www.digidesign.com/compato.
2. PCI Optimization for 33-64 Track Sessions with Pro Tools|24 MIX plus & MIX 3 Systems (Mac) Pro Tools|24 MIX plus & MIX 3 customers, (i.e. Pro Tools systems with at least one MIX Core and one
MIX Farm card) running large sessions, (33-64 tracks with high edit density, lots of automation data and/or large numbers of Plug-Ins), may experience "DAE -6042 - PCI Bus Too Slow"errors. These errors may occur due to the PCI bus being unable to handle the high volumes of "PCI traffic"required to play back the session. DAE 5.1.1 CS 1 improves Pro Tools PCI performance in these instances by placing the second 32-voice Playback Engine on the first MIX Farm card instead of
on the MIX Core card. Digidesign recommends that Pro Tools|24 MIX plus & MIX 3 customers regularly experiencing -6042 errors try this version of DAE, as it may reduce or prevent the occurrence of these errors with a particular session on a particular system, depending on the extent of PCI-bus overload.
Who Should Use this Update?
This update should be used only by:
- Any Pro Tools TDM or LE 5.1.1 Macintosh user wishing to add removable SCSI, hot-swappable SCSI or Firewire media to their system, without needing to quit and re-launch Pro Tools for correct recognition and operation.
- Pro Tools|24 MIX plus & MIX 3 system owners (i.e. with at least one MIX Core and one MIX Farm card) experiencing -6042 errors to alleviate the occurrence of this error.
Please Note:
- Pro Tools DAE CS 1 is provided is an interim "customer support (CS)"release to address the issues described. While our testing has not shown any problems to be caused by these changes, this version has not been fully qualified by Digidesign. For this reason, we recommend that you only install this version of DAE if you need the specific functionality it provides.

This error can sometimes be posted when a plug-in is attempting to be instantiated on a DSP chip which is not available.
What should actually be posted is an error such as " xxx plug-in could not be created. The required DSP is not available".
Remember that Mix Cards have three types of DSP chips:
-3 SRAM chips (used for effects such as short delays and reverbs".
-2 DRAM chips (used for effects such as long delays, chorus', and flangers)
-1 "no" RAM chip (used for effects such as eqs and some compressors).
If looking at the DSP Resources Window in Pro Tools, or the old Allocator software, the order of the chips from top to bottom (Detailed view) or left to right (Small view) is:
1 = SRAM
2 = No RAM
3 = DRAM
4 = SRAM
5 = SRAM
6 = DRAM
Read the the documentation for the plug-in you are using (or call the manufacturer) to find out which type of chip it uses, and then check to see if there is a chip of the type it needs available for usage. If there is not, then you need to either quit

1) The disk is corrupt - run Windows Scandisk or another disk utility. Initialize if practical / necessary.
2) One or more audio files in the session were deleted since the session has been open.

Make sure the Digidesign Direct I/O extension is installed in the extensions folder.
This will also happen if Pro Tools or any other application using DAE is also launched. Quit any applications using DAE and Direct I/O will be available in Soft Sample Cell.
