reboot your system and delete the Preferences from the system's preferences folder.

This can occur if you relaunch Pro Tools too quickly after shutting down. You should wait until disk activity has stopped (or wait a few seconds) after quitting Pro Tools, before relaunching Pro Tools.
One of the last thing Pro Tools does when quitting, is to SAVE the preferences. If you (re-)launch Pro Tools, one of the first things it will do is READ these preferences. If these two processes occur simultaneously, the system will not allow either process to complete.

There is currently a problem affecting D-Verb especially when using certain Algorithm settings. Room 2 suffers from this problem, although other algorithms could be affected as well. This problem is more common on Pentium 4 processors.
Workarounds:
1) Use a different Algorithm (e.g. Hall).
2) Insert a plug-in (e.g. POWr Dither) before the D-Verb.

This has been seen with the Waves 4.0 Rvox plug-in, possibly others.
Go to www.waves.com and hit "Downloads". Under the "Quick links" section hit "Updates" and download "Wave-Shell DAE 4.0.1 x".
2. Go to: HD>Library>Application support>Digidesign>Plug-ins – trash the current Wave-Shell 4.0_x and replace it with the new one (Wave-Shell DAE 4.0.1 x).
WaveShell-DAE 4.0.1_X fixes problems, such as loud noise bursts, occurring when issuing Play/Stop commands. If you are using Pro-Tools TDM/LE 6.0 or higher, it is highly recommended to use this WaveShell.
Installation: Go to Digidedign’s Plug-Ins folder (Your Hard Disk / Library/ Application Support/Digidesign/Plug-Ins), trash WaveShell DAE 4.0_X and replace it with WaveShell-DAE 4.0.1_X.

This is a known issue in Pro Tools 6.1, and is fixed in 6.4.
As a workaround a new DAE was developed to overcome this problem. Install DAE v.6.1cs1 (available on the 6.1 CD in the Additional Files/Beta Software folder, or as a download from http://www.digidesign.com/download/daedsi/).

This error can be caused when installing over a previously installed version of Pro Tools that used different Digidesign hardware (e.g. installing Pro Tools LE for Mbox over a previously installed version of Pro Tools for 001).
If you have both installation CD's for each type of hardware, run the uninstaller from those CD's, then restart your computer and do a reinstall of the proper version.
To uninstall from the CD, put the CD in your drive and double-click on the 'Install Pro Tools (LE)' icon from the CD.
Continue thru the license agreement and, when you get to the page that says 'Custom Install', click on 'Custom Install' and, from the drop down menu, select 'Uninstall', then click on the 'Uninstall' button in the lower right.
If you do not have both installation CD's, you can run the uninstaller from the CD you DO have, but you will also need to remove some components by hand, either by logging in as Root or by booting into OS9 (if your computer is capable of this) and removing the following:
>System>Library>CFMSupport - remove DAE, Digidesign DSP Manager, DigiDirectIO, DigiHardwareSetup, DigiHwGUI, DigiStreamManager and DigiSystemInterface
Then trash the DAE Prefs folder, Digisetup, and Pro Tools LE preferences in >Users>Current User>Library>Preferences
You should then be able to reinstall the proper version for your hardware.

Important: Read Abase # 27900 for more information, the solution there may solve this problem.
Do you have a Yamaha Motif keyboard? This problem can occur after installing the Yamaha software editor application, Motif Tools application or Tyros software. It is not clear if these application(s) must be installed to use the USB capabilities of Yamaha Motif, but it seems likely. Apparently, this software is preventing Pro Tools from launching and uninstalling it doesn't help Pro Tools to finish its launch.
Unfortunately, There is no solution to the problem at this time. Uninstalling the Yamaha software editor application or Motif Tools from the Add or Remove Program Control Panel does not fix this issue. Consequently, there must be elements of the Yamaha software that are left behind and aren't being removed when using the Add or Remove Program Control Panel.
Digidesign Tech Support can only recommend that the Pro Tools user call Yamaha and find out how to completely remove the software editor application. Otherwise, the only other choice is to backup the system hard drive and reinitialize (f-disk) and then reinstall Windows, Pro Tools, etc. Do not install the Yamaha software editor application again. Unfortunately, the Yamaha Motif might not be usable as a USB device and must be connected to a MIDI interface with MIDI cables instead.
