This is a known issue (35387), and is a design limitation with Pro Tools and the OS.

After installing Pro Tools v6.7 or v6.7cs8 with the Command|8 drivers, Command|8 is setup as a Controller in the MIDI Controllers Perhipherals dialog box along with "Receive From" and "Send To" but it then appears "Offline".
Please update the Command|8 firmware to version v02.01.02
Go to Program files / Digidesign / Command8 folder, launch the C8firmwareupdater application, select the file App_02.02.02.srec localized into this same folder and Update Firmware.

The command|8 is available in three configurations:
>> 90-120v for Japan and the USA
>> 220-230v for Europe
>> 220v for the UK

Please note: you must be in a darkened room in order to be able to see the following problem.
1. Create a new session.
2. Create one mono audio track.
3. Hit Play
4. Look at the blank line on the Command|8 scribble strip below the rolling counter.
Results: any vertical lines (like bar and beat separators) will be ghosted lightly on the lower strip.
Streaking shadow or vertical lines that make up the numbers continue down into the bottom row of Command|8 scribble strips in a lighter hue of orange.
LED display has "streaking and shadowing" effect where top line info bleeds down to bottom line.

The Command|8 audio control surface made by Digidesign is ONLY supported with the PC x.5.1 releases of Newscutter, Xpress Pro and Media Composer Adrenaline. Previous releases- and the MAC versions are not supported. However the Command|8 is supported with the MAC versions Pro Tools only.
For information on setting this up in your Avid environment- please see the attached guide.
Here are some troubleshooting tips- and helpful hints for using this device:
*Make sure Command|8 or 002 are turned on before launching Avid application.
*USB hubs and extenders will not work with the Command|8
*If your faders intermittently jump or jitter there is a firmware upgrade on the Digidesign website to fix this issue. From www.digidesign.com navigate to Support then to Command|8.
*Faders are heat sensitive- writing automation with a blunt object could cause damage to the faders
*If you are not seeing the Command|8 showing up as a MIDI Controller is either Avid or Pro Tools, reloading the driver from the CD will usually fix this issue.
*If the Command|8 is displaying values late, more than likely the user has a USB hub- which is not supported, removing it will fix the issue.
*NOTE: The Command|8 must have another piece of Digidesign hardware in order to launch Pro- Tools such as 888, M-box or the 002.
*Be aware the 002 should not run on the same computer as Adrenaline- both use Firewire connections. Command|8 can run simultaneously because of it's USB connection.

This behavior results when a Pro Tools MIDI track input and/or output is assigned to Command|8 port 1 on the Mac, or Command|8 Surface port on Windows XP. These ports are reserved for control surface communication and should never be assigned to MIDI track input or output ports.

This may occur after taking ProControl, Control|24, or Command|8 offline.
Bringing the control surface back online, closing and reopening the session, or toggling the plug-in to inactive/active should resolve the problem.

CC's 32-63 are reserved for double byte messages or LSB values for 0-31 so they're not available as individual parameters.
