In one case, this happened whenever an ADAT was connected, but powered OFF.
Powering ON the ADAT before opening a session resolved this behavior.

Soft SampleCell 3.1 currently has versions for LE and HD on the installer. If you install the LE version on a Digi 002 system, it will corrupt the Digi 002 DSI and other Pro Tools extensions.
However, the Soft SampleCell 3.1 installer for HD will work fine with the Digi 002.
To get your system up and running again, first uninstall Soft SampleCell 3.1 and uninstall Pro Tools LE 5.3.2; then reinstall Pro Tool LE 5.3.2 followed by installing the HD version of Soft SampleCell 3.1.
The Soft SampleCell installer will be updated, in order to prevent this from happening in the future.

This error can be caused by issues with QuickTime. Follow the instructions below for resolving this problem:
1) If Pro Tools LE 5.3.1 does not find QuickTime during initial installation, it will automatically take you to the Install Wizard. Allow the Pro Tools LE installer to continue and install QuickTime. Assuming it is successful, you will receive a Pro Tools LE 5.3.1 install completion message.
2) Restart the computer.
3) Before launching Pro Tools, open the Add/Remove Programs Control Panel and uninstall QuickTime. There will be 2 options, "Uninstall everything" or 'Uninstall." Choose "Uninstall everything." This will remove all software relating to QuickTime.
4) After successful uninstall of QuickTime, launch Pro Tools LE 5.3.1. you should be taken to authorization screen and enter your serial number.
%) Quit Pro Tools LE and proceed and reinstall QuickTime.

This has been seen with Type III Meridien cards and Pro Tools/AVoption|XL 5.1.3 and earlier software. Digidesign began shipping Type III Meridien boards with AVoption XL systems around June of 2002. Previously, Type II cards were included.
To run AVoption|XL with a Mix or D24 system, it is required that the AVoption|XL 5.3.1 installer be run. Specifically, the file Meridien24p.dll 5.1.3r2 must be installed in the System Folder/DAE Folder/Video Engine Folder, and the file SCRP0101.abf must be installed to the System folder/DAE/Video Engine/SupportingFiles/Drivers folder. If an AVoption|XL 5.3.1 installer is not available, these files can be downloaded from www.digidesign.com.
These components are also backwards compatible with Meridien II cards so there is no harm in installing them if you think you may have an older card.
The version of Avid System Test that ships with AVoptionXL 5.0.1 - 5.3.1 does not support the Meridien type III cards and thus, fail to initialize the card at launch. An updated version of Avid System Test will be made available in the future.
Meridien Type III can be identified by its 12 LED's across the top or by the Avid part number 0030-03138-01. This card should work fine with Pro Tools HD based systems, although Avid System Test vDiags_8.1_CPR_d4 which ships with AVOption XL 5.3.1 does not recognize the Type III cards and fails to initialize at launch.
Meridien Type II can be identified by its 10 LED's across the top or by the Avid part number 0030-03007-01. This card should function with all D24, Mix, and HD based AVXL systems.
